It started in conversations.

I talk to a lot of counselors, coaches, therapists, and doctors… it goes something like this:



Them: “Casey, I have a client I want to refer to you.”


Me:  “I am full right now.  I do have a waiting list if that's possible for them.”  


Them:  “I don’t get it. How do you only see two clients a day, two days a week?  I’d go broke and couldn’t pay for this office or my staff.”


Me:  “I know; I had to experience a crisis actually to make the changes.  It’s not for everyone.”


Them: “Yeah, OK, but how?”


Me: “When I got fired, I had to replace my income immediately.  There was no check coming the next month.  I had zero ramp-up time.”


Them:  “But that’s only four clients, right?”


Me:  “No.  I see my clients only once a month.  So I can see about 16 total.  They all pay a monthly subscription fee of $400 ($100 a week).  I want it to be affordable and about the same as a month of copays, as I don’t take insurance. While we only meet once, I give them four weeks of homework assignments which are automated on my site within the multimedia courses and automatic emails.  My site sends out monthly and quarterly online forms for them to complete, and I ask them to contact me (usually by text) between sessions.  There are more touch points when needed.  For example, some meet with me in a group format.  All sessions on video calls. 


The cool part is that I have had many clients stay in the program for over three years now.  Those don’t need to meet with me monthly anymore; they are content that I’m on ‘retainer,’ they have access to the coursework I’ve created, and occasionally they will text or email with a question or an issue I handle with a quick reply.  When they move to this ‘maintenance phase,’ I consider that slot open and admit a new client.  


If half of my caseload is in later phases of the program, I can easily have 32 total, as half are not taking session time.  My annual income is over $75K at that point.  

However, I have no overhead.  Barely any expenses. All clerical work is automated in a ‘set it and forget it’ manner.  But I also have some particular clients that pay more.  


Now let me ask you, what two months does your income go down?”


Them, “Easy, August and December.  Clients are no show or don’t schedule because of school starting and Christmas.  Summer gets lean too.”


Me: “Me too!  My clients don’t schedule as much during those seasons. Half of them skip those months. But since they pay a monthly subscription (like Netflix), my income stays the same, and I do less work, which is perfect because I have kids starting school and holidays myself.  My income has much less fluctuation now.”


Them:  “But it sounds like your clients are getting the short end here.  How do you think your clients do in your system?”


Me:  “The most common comment I get from clients is “This is the smartest system I’ve seen!”  They are getting much more for much less money and time investment.  From the comfort of their home or anywhere -with any internet-connected device, they get 24/7 access to dozens of course topics.  They never waste time in a waiting room or phone calls that take too long with receptionists to make or reschedule appointments.  


I’ve automated about 80% of what I used to do in-session.  Now we have the time to go further and faster in session.  It is much more productive.”


Them:  “How do you set this up?  Was transitioning your clients hard?  How do you… uh.  Will you help me?”


“Yes.”

In a post asking for help on developing content for groups I suggested:

Start with a curriculum. As time goes by you will modify and make it your own, customizing it to your particular client.  When covid started and took my private practice virtual only, I started building online courses to automate what used to do in person.
Every month I build out more and more topics and tackle additional issues. Incredibly I've built over 50 separate courses in less than a year! How did that happen?
I resourced all my old notes and plans from the last 20 years and updated them, make a short video with handouts, worksheets, and homework.
But they all came from a starting place, where you are now. I can share mine with you as a starter-pack, but you will still have to make them in your own voice eventually. Which is a fun process!

Wow, that's a truly impressive jump-start! I'm looking to do the same, and I've got a ridiculous amount of personal writings, notes, etc, but get hung up trying to break things down into a set program - everything feels so vital and intertwined that when I start putting something together it grows into something gargantuan.  If there's anything you're willing to share or suggest, that'd be greatly appreciated!

I would be happy to help.  I've assisted so many clinicians that I have a system for it.  

And I felt the very same.  I did have another kind of 'jump start' with all this.  Two years before COVID I started an online membership program in my other world:  art.

Now my worlds of Art + Therapy are both online.  

Back then I was really into a weird and fun form of art called acrylic paint pouring (also called fluid and flow art)  I started teaching it and it was paying so well that I started a paid membership program for other teachers of it all over the world.  Even now (when no one is teaching in person) it's a significant income stream for me and my family.  

So I had the opportunity to PRACTICE the subscription model with art.  For nearly a year now I've done the same thing but with my clients.  They all pay a monthly fee and get access to me.  (Hey, that rhymes.  I'll use that!)   

I already had the course/membership platform for the teachers of art, therefore I had practice turning knowledge into a highly specialized online courses.  

I hear you as far as where to begin and end an information product.  It does have to be focused and concise.  As you create, think "What is the one problem I can solve for my client immediately as they read this?"   Then address ONLY THAT.  You can always make additional products to expand or be the natural next step.  Don't try to pile it all in one.  (BTW your problem is perfectionism --ask me how I know about that!  Ha!)

People spend an average of only about 20 minutes on such activity.  I make my video presentation as a Live (no fancy editing) about 6 minutes long.  (Yep! the short attention span of the viewer makes this ideal)  Or break them down into shorter videos of a max of 6 minutes. 

Then very accessible usable worksheets they can do in  5-10 minutes.  Most of mine now are google forms that I use a Chrome app called Form Publisher to automatically send me and them a copy in an email and populates into my G-suite and becomes part of their chart.  So easy!  (Yes, I have a BAA signed with Google.)

I've automated about 80% of what used to be in-session work this way.  It energizes me to think of what  I can automate next!
